The family are worried about the brother. They don't think he has long left. He is lost in his addiction but can't live life without this medication. Being clean and sober is not about just giving up, we have to find out the 'why'. Why do they need this medication? Why are they struggling to do what others find easier? Can we be more empathetic, just enough to allow us to see the person and not the medication?
